Mercy Health Network has received a $10.1 million Health Care Innovation Award from the U.S. Department of Health and Human Services for an initiative to build a better health care delivery system for rural hospitals and clinics in Iowa and Nebraska.
The grant will benefit 25 critical access hospitals and 73 clinics in the two-state area that are affiliated with Mercy Health Network member hospitals, including Mercy Medical Center - Des Moines.
The funding "will support our continued efforts to develop targeted and innovative methods of care by using information on the overall wellness, chronic conditions and health care needs in rural Iowa," Mercy Health Network President and CEO David Vellinga said in a release.
More than 160,000 people could see benefits through better management of chronic conditions, such as heart disease, high blood pressure and diabetes. The project will use an in-clinic health coach model developed by Mercy Clinics in Des Moines.
