Hy-Vee Inc. announced Tuesday that it is now a joint owner of Exemplar Care as it seeks to provide more affordable primary care and urgent care services to Iowa patients and employers.

As part of the agreement, Exemplar Care clinics – located in West Des Moines and Ankeny, with a new one coming to Bondurant – will be renamed Hy-Vee Health Exemplar Care. Hy-Vee Chief Medical Officer Dr. Daniel Fick and Exemplar Care’s leadership team, including Dr. Jon Van Der Veer, will manage the clinics.

Employers using Hy-Vee Health Exemplar Care can provide their employees access to unlimited primary and urgent care through memberships for a fixed monthly fee or layer the membership with a major medical health plan.

Health care memberships are also available to individuals and families outside of an employer-funded plan.
