Primary Health Care receives national recognition from CMS

The Centers for Medicare and Medicaid Services recently awarded Primary Health Care in Des Moines “exemplary status” for improving patient experience and outcomes, controlling population health and reducing costs through the CMS Transforming Clinical Practice Initiative. The work of the PHC teams to provide innovative health care solutions through public school and homeless shelter outreach, the use of a mobile health unit, and HIV care and prevention services were all recognized by the agency in an award letter. Launched in 2015, the CMS Transforming Clinical Practice Initiative is a national four-year technical assistance program designed to help clinicians achieve large-scale health transformation in primary and specialty care practice settings. “I am proud of the ways that our team goes above and beyond to serve patients in Central Iowa,” PHC CEO Kelly Huntsman said in a release.
